Making an appointment for checkup/receiving appointment confirmation by mail

Within one week after the checkup appointment is made, we will send you an appointment confirmation by mail.

Receiving a checkup kit prior to the checkup date (pre-check questionnaire, sample containers, etc.)

We will send you a checkup kit one or two weeks before the checkup date. The kit includes detailed information about the checkup package, as well as a pre-check questionnaire and sample containers, which are to be filled in advance and brought to the checkup.

Receiving the checkup

1. Signing in (submit the completed pre-check questionnaire and filled sample containers)
2. Taking measurements of height, weight, blood pressure, etc.
3. Ophthalmologic examination (vision test, intraocular pressure measurement), hearing test

Receiving the checkup result by mail

You will receive a detailed checkup result by mail approximately three weeks after the checkup.
The checkup result may include suggestions for further testing or treatment.

Notes about taking a checkup

1. The test items listed above are our standard checkup package.
   If you wish to receive any other types of test in addition to the ones listed here, contact us by telephone or fax to let us know details of such additional tests.
2. Corporate checkup is a medical checkup package for your existing or new employees based on the Industrial Safety and Health Act.
3. The fee and the test items for a corporate medical checkup may differ depending on your contract with the health insurance association.
4. The total fee will not change if you choose not to include some of the test items in the package.
5. The checkup result will be summarized into a list and sent to you by mail at a later date.
   Note: Interview with our physician or dietitian is available upon request at a later date (by appointment only).
6. We neither have our own parking facilities nor offer discounts for nearby parking services. Please do not come to the clinic by driving a car.
7. You may be unable to sign in if you are late for the appointment time. Make sure to come to the clinic before the appointment time.
