The Department of Gastroenterology at Showa University Fujigaoka Hospital conducts diagnosis, treatment and research on benign and inflammatory diseases as well as malignant diseases in the digestive tract (esophagus, stomach, duodenum, small and large intestines), liver, gall bladder, and spleen in general. In particular, we actively employ treatment endoscopy, including endoscopic examination, diagnosis with upper and lower gastrointestinal endoscope, enteroscopy, capsule endoscopy, endoscopic ultrasound (EUS), endoscopic ultrasound-fine needle aspiration (EUS-FNA), endoscopic submucosal dissection (ESD) of the esophagus, stomach and large intestine, endoscopic bile duct stone treatment, and EUS-guided interventions. We also conduct emergency procedures using an emergency endoscope such as emergency biliary drainage for gastrointestinal hemorrhage or acute cholangitis in coordination with the critical care center. In addition, we started providing diagnosis and treatment under the Digestive Disease Center in cooperation with the Department of Gastroenterological Surgery in 2010, and have established a system of close cooperation between internal medicine and surgery. Especially regarding diagnosis and treatment of cancer, the Departments of Gastroenterology, Gastroenterological Surgery, Medical Oncology and Palliative Medicine, and Radiology jointly hold a Cancer Board meeting every week to determine and practice the best treatment policy.

As society continues to age, the number of people suffering from not just one disease but various complications is increasing. The treatment needs of patients are also changing, with some patients not wishing to have invasive tests or treatments and some preferring palliative care even when the cancer is resectable. Under such circumstances, the need for minimally invasive endoscopic diagnosis and treatment has increased, and is expected to continue to increase. General and Gastroenterological Surgery

The Department of General and Gastroenterological Surgery, Showa University Fujigaoka Hospital provides diagnosis and treatment of a wide range of diseases, including malignant digestive system tumors (esophageal cancer, gastric cancer, small intestine cancer, large intestine cancer), malignant diseases of the liver, bile duct and spleen (liver cancer, metastatic liver cancer, bile duct cancer, gallbladder cancer, pancreatic cancer), benign and acute diseases in the abdominal area (peritonitis, appendicitis, intestinal obstruction, intra-abdominal organ injury, etc.), and general surgical diseases such as hernia. While surgical therapy is our main method, we seek to provide better medicine by employing various techniques, ranging from laparoscopic surgeries that are easier on the body to highly difficult surgical operations on extremely advanced malignant tumors.
In addition, starting in 2020, the Department of Gastroenterology and the Department of Gastroenterological Surgery perform diagnosis and treatment under a cooperative system as the Digestive Disease Center, which was established as an in-hospital center.
In diagnosis and treatment of cancer, the Department of Medical Oncology and Palliative Medicine, the Department of Radiology and the Digestive Disease Center hold Cancer Board meetings to determine and practice the best treatment policy.

The characteristic of our department is neurosurgery medicine centering on neuroendovascular therapy.
Dr. Terada, who is the Department Director, has successively served as a medical advisor and board member for the Japanese Society for Neuroendovascular Therapy, a board member of the Japanese Society of Endovascular Intervention, a management committee member and medical advisor of the Japanese Society on Surgery for Cerebral Stroke, in addition to important posts in international societies such as LINNC and ALICE. We take pride in him as one of the leading neuroendovascular surgeons in Japan. He has personally conducted more than 3,000 intravascular treatments, and difficult cases are referred to him from all over the country, not just the Kanto region.
Furthermore, seven staff in our department have qualified as neuroendovascular therapy specialists in addition to being neurosurgery specialists, making our department one of the largest neuroendovascular therapy centers in Kanagawa Prefecture.

Internal Medicine (Diabetes,Metabolism and Endocrinology)

The Department of Internal Medicine (Diabetes, Metabolism and Endocrinology) mainly handles diabetes and endocrine diseases, particularly of the thyroid, pituitary gland and adrenal gland. The entire department works together to provide the highest level of care possible for any disease. We also aim to provide comprehensive measures to solve various problems of patients with diabetes or endocrine diseases.

The basic policy of the Department is to provide diagnosis and treatment from the standpoint of the patient. We try to provide a careful explanation, treatment based on the wishes of the patient, etc. As part of such efforts, we make sure to suggest not just our hospital but multiple other hospitals which we consider to be good for the patient and allow the patient to choose, as surgery may often be required for some endocrine diseases. Even for diabetes, we ask about the lifestyle and views of the patient, and propose whatever treatment we consider is best for the patient. Treatment is not a top-down action from medical workers, but a joint effort with the patient.

While the staff of our Department have various career backgrounds ranging from experienced to mid-level to young, we are constantly making improvements in order to meet everyone’s expectations under the above policy. There are also many doctors who are qualified as both diabetes specialist and endocrine disease specialist, and everyone, from the experienced to the young, provides care in close cooperation.

Internal Medicine (Nephrology)

The types of diseases handled by our Department vary widely, including chronic renal diseases such as glomerular nephritis, various renal diseases such as acute and chronic renal failure, blood purifying treatments centering on blood dialysis and peritoneal dialysis therapy, electrolyte imbalance, renal complications of high blood pressure or diabetes, etc.
Prevention to stop the disease progressing or advancing into a serious state is especially important with kidney diseases, and we practice treatment and prevention management by actively introducing tonsillectomy combined with steroid therapy for IgA nephropathy, and diet therapy for chronic renal diseases. We have also established chronic renal disease outpatient services to provide information on chronic renal diseases and select a renal replacement therapy, and special outpatient services for polycystic kidney. For renal diseases as complications of rheumatism or collagen disease, we provide treatments in close coordination with the Department of Rheumatology.
We will continue to provide high-quality medicine and contribute to medicine for the region in the future.

Internal Medicine (Medical Oncology and Palliative Medicine)

In recent years, the number of deaths caused by malignant neoplasms and cancers has been increasing, and we have entered an era when one out of two Japanese citizens develops some type of cancer, and one out of three dies of cancer. Especially when metastasis from the primary lesion is observed in a distant part from the outset, or when the cancer recurs after surgery, the treatment results are not always satisfactory. The Department of Medical Oncology treats such cancer patients mainly with chemotherapy; in other words, it is a specialized department for anticancer drug treatment.

In Japan, medical oncology departments are gradually becoming more common in university hospitals and general hospitals, in addition to hospitals specializing in cancer. Having been founded in October 2014 and reorganized as the Department of Internal Medicine (Medical Oncology and Palliative Medicine), the Department of Medical Oncology at Showa University Fujigaoka Rehabilitation Hospital is a relatively new department among the internal medicine departments of Showa University Fujigaoka Rehabilitation Hospital.

Our department makes the following promises to our cancer patients:
  1. We will try to achieve the best level of treatment results in Japan through the provision of chemotherapy consisting of the latest standard of care.
  2. We will try to provide cancer therapy with a high degree of satisfaction for the patient through the provision of excellent team medicine in cooperation with doctors of other related departments such as surgical oncologists, radiation oncologists, and palliative doctors, nurses, pharmacists, nutritionists and social workers, in addition to medical oncologists.
  3. We will promote the development of next-generation therapies through the provision of experimental therapies, which typically involve clinical trials.

Furthermore, with regard to medical education, we will actively accept young doctors who aim to become medical oncologists as well as provide education for the students of the School of Medicine, and help them to acquire qualifications as specialists and academic degrees through medical research. We accept doctors who wish to receive training, and medical oncologists who want to work with us as instructors at the School of Medicine, from all over the country and at any time.

Our Department handles skin diseases in general; please consult us for any type of skin disease. We also have a system for handling intractable skin diseases that require specialized diagnosis and treatment as well as unique therapies.

Our unique therapies include low-pain treatment of ingrown nails (treatment at the patient’s own expense: wire method), phototherapy for psoriasis, atopic dermatitis, intractable alopecia areata, etc. (we have introduced the latest full-body narrowband UVB phototherapy device), bleomycin hydrochloride local injection therapy for verruca vulgaris (own expense), SADBE local drug treatment for alopecia areata (own expense), and carbon dioxide laser therapy for benign tumors.

Breast surgical oncology

The morbidity of breast cancer is increasing by the year, causing growing social concern. In the future, the number of people receiving medical checkups is expected to increase, as well as the number of people who will need detailed examination or treatment. The Breast Center of this Hospital has image diagnostic devices, examination rooms and treatment rooms in addition to consultation rooms, so that patients can receive the series of diagnosis and testing quickly after changing into the special examination gown.
We also provide second opinions (by reservation), and have a system for putting patients diagnosed with breast cancer at ease when they receive treatment.
The treatment of breast cancer requires a long period of hospital visits and follow-ups. We provide diagnosis and treatment in close coordination with other doctors in the region.

Spine Surgery
We specialize in surgery to (1) relieve nerve compression and (2) maintain or reconstruct the support of the spine, which is the foundation of the body. Since spinal disorders cover a wide range of areas from the craniocervical spine to the pelvis, surgery must be performed using a variety of methods, from localized surgery such as herniated disc surgery to major surgery that considers the balance of the body, such as spinal deformity. In addition, each patient has a wide range of ages and underlying diseases, from children with congenital diseases to the elderly with underlying osteoporosis, and each patient has different needs for attention. I believe that the most important thing is to think about what one's own family would do for each individual patient and strive to do the best we can. If there is no other way to improve the patient's disease other than surgery or if surgery is significantly superior to other treatment methods, we will choose the surgical method that is best suited to each patient and devote ourselves to that surgery. We are also deeply committed to orthopedic education and research. The division has four key priorities:

Trauma and Fracture Surgery Education
 
As a national leader in resident education, the Department of Orthopedic Surgery considers resident training one of our most important objectives. The trauma-trained surgeons in the Division of Trauma and Fracture Surgery guide residents in the treatment of common fractures and the stabilization and treatment of complex injuries.
 
Members of the division provide orthopedic-related trauma education through didactic lectures, state-of-the-art simulation, and direct teaching of operative skills that enhance the learner’s ability, knowledge base, and dexterity.
 
Our academic curriculum includes a weekly didactic fracture conference, and each clinical site maintains its own academic schedule and case conferences. Our faculty also oversee resident skills sessions throughout the year in both large and small groups.
 
Residents participate in rotations as part of the Department of Orthopedic Surgery’s residency program. All interns attend the AO basic fracture course to gain further fundamental knowledge of operative treatments for fractures.
 
We provide reconstruction using the Ilizarov method, deformity correction, limb lengthening, osteomyelitis surgery, and soft tissue reconstruction using free flap for severe extremity trauma and post-traumatic sequelae (deformity, shortening, osteomyelitis, contracture, infected pseudoarthrosis) that are difficult to treat, and we accept referral patients for treatment. Reconstruction of both skin and bone requires treatment that crosses the boundary between plastic surgery and orthopedics. Showa University Fujigaoka Hospital is one of the few hospitals in Japan where both the Ilizarov procedure and microsurgery can be performed by the same physician.
 
Shoulder & Elbow Surgery
Shoulder & Elbow Surgery Group, led by Professor Naoya Nishinaka, provides numerous outpatient consultations, hospitalizations, and surgical treatments at both Showa University Fujigaoka Hospital and Showa University Fujigaoka Rehabilitation Hospital. We specialize in shoulder and elbow joints, sports injuries and disorders, and degenerative diseases, and provide numerous outpatient consultations, inpatient admissions, and surgical treatments.
In orthopedic surgery, especially in joint surgery, rehabilitation centered on exercise therapy is a very important factor that determines the outcome of treatment, regardless of whether surgery is performed or not. The upper limb team, in particular, works closely with physicians and rehabilitation specialists. Especially in the upper limb group, physicians and rehabilitation staff regularly update their knowledge at study sessions and conduct academic activities and research together.
In addition, for many years in the past, we served as the medical check-up physician for the Chiba Lotte Marines. In recent years, we have been making efforts in the field of research and education, including collaboration with the Institute of Sports and Exercise Science. He also contributes to community medicine by serving as the physician in charge of elbow checkups for youth baseball players in Yokohama, Japan.
 
Knee Reconstruction and Replacement Surgery
Knee Joint Group provides outpatient care as well as inpatient and surgical treatment at both Showa University Fujigaoka Hospital and Showa University Fujigaoka Rehabilitation Hospital. The target diseases include sports injuries, disorders, and degenerative diseases, and we treat knee joint diseases in general. For knee ligament injuries, we focus on surgical treatment for anterior cruciate ligament injuries with the goal of early return to sports. For meniscus injuries, we actively perform meniscus suture using a knee arthroscope to repair the damaged area. We also perform autologous cultured cartilage transplantation for cartilage injuries. We also actively use exercise therapy and dynamic insole (plantar plate) therapy to treat pain around the knee caused by overuse due to sports. For knee osteoarthritis, we provide surgical treatments such as knee joint replacement and high tibial osteotomy.
 
Division of Hip Reconstruction and Replacement Surgery
The hip joint group is currently composed of Lecturer Minoru Watanabe, Assistant Professor Tsubasa Ishikawa, and two other physicians who specialize in hip replacement and revision hip arthroplasty.
The group handles a wide range of hip joint diseases, including adult hip joint diseases such as osteoarthritis secondary to acetabular dysplasia, which occurs more frequently in Japan than in Europe or the United States, and pediatric hip joint diseases such as necrosis of the femoral head, developmental hip dislocation, Perthes' disease, and slipped femoral head.
Surgeries are performed mainly on Wednesdays and Fridays, with approximately 200 to 300 cases per year, ranging from joint-sparing surgeries (acetabular labioplasty, femoral head rotation osteotomy, and kyphotic osteotomy) and hip replacement (cemented or cementless, mainly for anterior entry, with some cases posterior entry) to revision hip arthroplasty for children and adults. The most common operations are hip replacements. In hip replacement surgery, which is the most common surgical procedure, we are committed to minimally invasive surgery according to each case, and we conduct daily medical care and research in pursuit of "Forgotten joint" (patients are not conscious of the operated joint in their daily lives).
 
Division of Foot & Ankle Surgery
For a long time, our specialty was limited in the field of foot surgery and only some diseases were treated surgically. 35 surgeries were performed in FY 2019, but the number is gradually increasing. The field of foot surgery is one that has not yet been fully explored and will continue to develop in the future, and we are making efforts in academic conference activities and writing papers, including overseas.
The head of the Foot Surgery Group, Dr. Murakami, studied at Nara Medical University, one of the leading institutions in the field of foot surgery in Japan, for one year in FY2018, and has been working at our hospital since FY2019, providing surgical treatment, orthotic treatment such as insoles, and drug treatment such as injections. For patients who need rehabilitation treatment, we refer them to a nearby clinic for rehabilitation.
 
Division of Pediatric Orthopedic Surgery
Emeritus Susumu Saito at the time of our hospital's opening, and has a long history, including serving as president of the Japanese Society of Pediatric Orthopedics. We mainly treat hip joint diseases including congenital hip dislocation, congenital club foot and lower limb diseases. We are the first department in Japan to introduce radiation-free ultrasonography, and Dr. Ogitani and Dr. Sekihara and Dr. Ito are also lecturers at the Infant Hip Seminar held by the Japanese Society of Orthopaedic Ultrasonography. While there are few hospitals specializing in pediatric orthopedics, not only clinics but also general hospitals, our clinic plays an important role in the field of pediatric orthopedics in the northern Yokohama area. In addition, we have pediatric orthopedic specialists at Showa University's other affiliated hospitals, Showa University Northern Yokohama Hospital and Showa University Koto Toyosu Hospital, with whom we share information and provide medical care on a daily basis.

Plastic and Reconstructive Surgery

Since 1968, the Department of Plastic Surgery at Showa University has grown, with Showa University Hospital in Hatanodai, Shinagawa, Tokyo as our base. Today, every affiliated hospital of Showa University has a plastic surgery department.

Having had Showa University Hospital as our base for nearly 50 years, on October 1, 2017 we started a new phase at Fujigaoka Hospital, which is an affiliated hospital. In addition, the Cleft Lip and Palate Center, which had been located within the Department of Plastic Surgery, was upgraded to the Showa University Cleft Lip and Palate Center as an organization under direct control of the President of Showa University, and started providing diagnosis and treatment with Fujigaoka Hospital as its base. We have also increased the number of doctors in charge to provide sufficient care to our patients.

All the medicine we had provided at Showa University Hospital before is now provided here at Showa University Fujigaoka Hospital. This move may be inconvenient for some people, but we hope you will continue to support us as all the doctors at Plastic and Reconstructive Surgery of Showa University Fujigaoka Hospital will keep working together to diagnose and treat our patients.

Obstetrics and Gynecology

Since its foundation in 1975, the Department of Obstetrics and Gynecology, Showa University Fujigaoka Hospital has grown in coordination with Showa University Hospital (Hatanodai), and Northern Yokohama Hospital (Tsuzuki, Yokohama), and with the support of local doctors and patients as an affiliated hospital of Showa University. As there are now fewer births and fewer facilities that handle deliveries, we have established a system for handling a wide range of obstetric and gynecological diseases in a 24-hour system as a core hospital in the region.
8 out of the 13 full-time doctors are specialists in obstetrics and gynecology, and each doctor is highly skilled in diagnosis and treatment while utilizing their specialty. As a medical education institute, we have been certified as a training and instruction facility by the Japan Society of Obstetrics and Gynecology as well as for perinatal care and tumor-related care, and are committed to education.
