生理学講座生体調節機能学部門では呼吸の神経機構を中心に多彩なテーマで研究を行っています。動物とヒト(患者を含む)を対象にし、電気生理学や脳イメージングなど多角的なアプローチで進めています。また多岐に渡る診療科の臨床医が大学院生として研究に参加しています。多くの学外研究者との共同研究も積極的に進めており、出身分野を問わず若手研究者の育成に取り組んでおります。

摘出脳幹-脊髄標本を用いた各種中枢神経機構の研究

1. 呼吸リズム形成の神経機構の解明
主に、ラット(またはマウス)の摘出脳幹-脊髄標本において、電気生理学的方法、光学的測定法などを用いて、呼吸リズム形成の神経機構を明らかにする研究を行っています。転写因子Phox2bが呼吸リズムジェネレーターである傍顔面神経呼吸ニューロングループ(pFRG)の特異的マーカーであることを明らかにしました。Phox2b発現ニューロンが光感受性チャネルの一種であるチャネルロドプシン2を発現するラットを作成し、呼吸中枢の局所ネットワーク解析,二酸化炭素受容および低酸素受容機構などを解析しています。
2. 交感神経活動の調節機構の解析
視床下部、延髄及び脊髄交感神経系中枢の神経機構の解析を行っています。
3. 呼吸筋収縮力の調節機構の解明
呼吸は横隔膜や肋間筋など多くの筋が適切に収縮することにより肺を換気しています。この筋収縮力の調節に関わる脊髄介在ニューロンの解析を行っています。
4. 呼吸と歩行の協調機構の解明
肋間筋や腹壁筋など体幹を支える筋は呼吸や歩行、姿勢制御など複数の役割を持っています。呼吸中枢や歩行中枢によるこれらの筋収縮力の調節機構を解析しています。
5. 吸啜、嚥下の神経機構の解明
我々は孤束核にあるPhox2b発現ニューロンが吸啜および嚥下のリズム形成に重要な役割を果たしていることを明らかにしました。吸啜、嚥下に関与する脳幹内ネットワークの解析を行っています。

ヒトの呼吸、情動、嗅覚の連関性の解明 —脳形態•機能画像解析を用いて−

呼吸は情動と嗅覚に深く関連しています。その連関性を磁気共鳴画像(MRI)、機能的磁気共鳴画像(fMRI)、脳波、光トポグラフィー(NIRS)の脳画像解析を用いて研究を進めています。現在3つのプロジェクトが進行しています。
1. コミュニケーションとしての呼吸
情動の変化は呼吸の変化として表れます。またヒトの情動に共感して呼吸パターンが他者と同調することを報告してきました。呼吸の社会相互作用、呼吸のコミュニケーションを課題に工学、心理学、医学、芸術など他分野と連携し研究を進めています。
2. 呼吸と嗅覚の連関性の研究
嗅覚は呼吸と深く関連しています。その連関性をfMRIを用いて活動部位の同定、活動部位間の接続性から検証しています。特に香りによる自伝的記憶の想起を課題に産学連携体制で研究を進めています。嗅覚は記憶とのつながりが深いため、嗅覚障害は記憶、認知の低下を示すことを報告してきました。嗅覚障害を初期症状とする認知症、パーキンソン病の脳機能、形態の異常を嗅覚低下から予測し得るスクリーニング法を目指し、現在昭和大学神経内科と共同研究を進めています。また近年、嗅覚と記憶について動物実験とヒトの実験を平行して行い、嗅覚刺激における海馬の神経再生の促進と速度を検証し、嗅覚刺激による脳機能トレーニング法の創出を目指しています。
3. 安静時呼吸と脳活動の研究 
呼吸は延髄でリズムが形成されているが、情動によっても変化し、また随意的にも変化します。これまで呼吸を変化させることで情動をコントロールできることを示してきました。MRI下で呼吸、心拍、体内CO2濃度を同時記録するシステムを確立し、安静時fMRIへの生理ノイズの影響を検証するとともに、脳内の活動と呼吸パターン、情動のつながりを検証しています。

中枢神経系リハビリテーションに関する研究

脳卒中をはじめとする中枢神経損傷患者は、運動、感覚、認知機能などの障害が残存しADLとQOLを大きく低下させます。これらの機能障害に対して、リハビリテーションを行うことで機能回復に結びつくことが分かってきました。しかし、リハビリテーションの効果や効果的なリハビリテーションの手法等、未だ不明点が多くあります。そこで、中枢神経損傷モデルマウスを作製し、リハビリテーションの効果について、生理学的、運動学的、組織学的、そして分子生物学的手法を用いることで研究を進めています。
